APPLICATION INSTRUCTIONS


APPLICATION CHECK LIST: This list will guide you through the steps necessary to complete your application.

ON-LINE APPLICANTS - You should have all personal information prepared before beginning the on-line application process.  This process must be completed in one setting.  Allow yourself enough time to do so.  Information necessary includes, but is not restricted to: previous school contact information, military information, previous employee contact information, reference contact information (pastor, church elder, and Christian friend). 

*NOTE: All forms and correspondence should be mailed to: Director of Admissions, Faith School of Theology, 29 Main Road, Charleston, ME 04422.

  1. Completely fill out the application.  (Click Here to fill out on-line application)
    _____ Include a small photograph of yourself. (if using on-line application, please mail separately)
    _____ Include a $35.00 non-refundable application fee. (if using on-line application, click here to pay on-line, or send payment with all other application materials.)
    _____ Include your handwritten essay.
  2. Reference Forms:
    _____ Ask your Pastor to complete and return the Pastor's Reference Form to the Director of Admissions.
    _____ Ask your Sunday School Superintendent or Sunday School Teacher to complete and return their Reference Form to the Director of Admissions.
    _____ Ask a Christian Friend who knows you well (other than as a student) to complete and return their Reference Form to the Director of Admissions.
  3. Transcript Request Forms:
    _____ Complete one transcript request form and mail it to your high school counselor.
    _____ If you have attended any college, we will also need your college transcripts. Complete the second transcript request form and mail it to the Registrar of that college.
    _____ Veterans, please have a copy of Form DD295 forwarded to Admissions office. Completion of the current semester is not necessary before sending transcripts.
  4. Medical Forms:
    _____ Certificate of Medical Examination. Have your physician complete the medical form and return it to the Director of Admissions.
    _____ Immunization Record. Please send a copy of your immunization record to the Director of Admissions.
